The Keys to Good Documentation

Effective communication is critical to our advocacy. Optima allows our advocacy teams to update each other regarding the case in real time which is why it is so important to make sure we are documenting our notes effectively. Our contact logs and hearing notes need to be clear and provide the reader with an understanding of the current status, any outstanding needs and issues discovered as a result of the action being documented. 

Optima serves as the child's case file and needs to be documented in a professional manner with accurate and updated information that is entered timely. As a party to the dependency case, we are required to comply with Requests for Discovery which includes information documented in Optima therefore, it is critical to ensure our communication complies with our 5 Cs and is something we would be proud to share with others.
